What we do

Our Legacy Services

Basic Home Preparation

We help prepare an individual’s home ahead of their return from hospital, ensuring it is safe, warm, and ready to support a comfortable and secure recovery.

Befriending & Companionship

Our volunteers provide regular visits or phone calls to offer conversation, reassurance, and social interaction, helping individuals feel supported and less alone.

Shopping or Essential Errands

We assist with grocery shopping, prescription collection, and other essential errands to ensure individuals have access to the necessities they need during recovery.

Food Parcel Support

For those facing short-term hardship, we provide essential food parcels to ensure no one goes without during vulnerable periods.

Emotional Reassurance

We provide compassionate conversation and reassurance to help reduce anxiety, restore confidence, and ease the transition back into independent living.

Digi buddy service

Our Digi Buddy service helps individuals learn how to use phones, tablets, and online services, enabling access to appointments and benefits.

Signposting to Additional Services

We connect individuals with relevant local services, healthcare providers, and community resources to ensure ongoing support beyond our immediate assistance.

Welfare Checks

We carry out friendly welfare visits or calls to monitor wellbeing, offer reassurance, and identify any additional support needs during recovery.

Sitting Service

We offered short sitting visits to provide companionship and give carers or family members short breaks when needed.

    MM

    Shopping support

Because I am completely bedbound, getting the right food in was really difficult. Home From Hospital Care collected my shopping list each week and bought the items I actually wanted, not just the basics. Having someone take time to chat with me also helped, especially as I no longer speak to my family.

      Supported Shopping Trips

I had not been out of the house for months and felt extremely nervous about going out again. With support, I managed to go to the local shop and later even the post office, which felt like a huge step. I felt happier afterwards and was very grateful for the help.

        Benefits and Practical Support

I was very stressed and felt like I had no help after losing my sight. Someone visited me, explained the support that might be available, and helped me contact my doctor so my needs could be assessed properly. I appreciated having someone take the time to discuss everything with me.
